Call to Order: Holly Durham, Chair, called the meeting to order at 3:35 p.m.

Minutes: The Committee reviewed and approved the September 16, 2014 meetingminutes.

Benefits Office Updates: Joe Williams, HR Benefits Director

Annual Enrollment: If you are enrolled in the 80/20 or the Consumer Directed Health Plan and wish to retain your lower monthly premiums, you must complete your wellness activities online again by October 31, 2014. Failure to complete these by October 31, 2014 will result in higher monthly premium costs for 2015 coverage.

There are no rate increases for 2015.

TO COMPLETE YOUR WELLNESS ACTIVITIES(Health Assessment and Smoking Attestation) follow the instructions below:

Go to theMyPack Portaland log in. Then click on FOR FACULTY & STAFF at the top in the black bar. In the center of the page, you will see Employee Self Service. Click on “Enroll in Benefits” and you will be logged in to the online enrollment system. At the far left, under the QuickLinks, click on “eEnroll-Click Here to Enroll”. This will put you into the online enrollment system.

Once in the enrollment system, click on the Benefits button with the red and white cross. You should see “2015 SHP Medical”. Click on the View/Edit Information button and if you have the 80/20 plan or the Consumer Directed Health Plan (85/15), you can click on “Premium Credits-Edit” to list a primary care physician, do your health assessment and complete the smoking attestation.

*Please note the smoking attestation is separate from the health assessment.

**Make sure you hit the Next button and ultimately the Save button as you go through these.

Modification to the dental plans for 2015 is the waiting periods. There are no longer waiting periods for dental plans except for orthodontic benefits.

 

Same Sex Marriage: With the recent change insame-sex marriage laws, those same-sex partners marriedpriorto October 13, 2014, may use the October 10, 2014 date as themarriage date. This30-day window to enroll will continue through November 12, 2014. After this enrollment window closes, you will have to wait until the next annual enrollment to enroll your same-sex spouse unless a qualified life event occurs such as loss of other health coverage, etc. If you get married after October 10, 2014, same-sex marriage will continue to be a qualifying life event. You will have 30 days from the date of the marriage to add your spouse to your coverage. The effective date of coverage will be the 1st of the following month.

The HR Benefits Office will need a copy of the marriage certificate/license to approve this life event.

Affordable Care Act (ACA): For all non-benefits eligible employees or temps hired on or after October 1, 2014. Our Standard measurement period will be October 1, 2013 – September 30, 2014

  • We will use a 12 month look back period to determine if the temp/employee averaged 30 or more hours per week over the prior 12 monthperiod (measurement period). If the person averaged 30 or more hours per week, health coverage must be offered at the beginning of the 13th month.
  • Ifeligible and the person accepts the health coverage, it must last for at least 12 months (the stability period) regardless of hours worked(or when the employmentends). After these 12 months of coverage, if the person is working less than 30 hours per week, the coverage may be cancelled.

National Saving for your Future: Lunch and Learns have been scheduled for the week of October 20 – 24th at various locations across campus.

Additional events are being planned for upcoming months. (Ex: Hot chocolate & Donuts, Breakfast, Pizza)

Nick Drake, Assistant Director Fitness and Wellness

Julia Buchanan and Nick Drake from University Recreation spoke about their efforts to get more wellness programs available across campus.

DASAFit- A Lighter State is one program currently in progress. A Lighter State is designed to be a weight loss and/or weight management program for DASA employees. It serves as both an incentive and accountability program for employees to stay active and engage in healthy behaviors. Through regular weigh-ins, weekly wellness objectives, and education sessions, participants will gain an understanding of the basic components of healthy weight loss and maintenance.
